Bariatric Surgery Pre-Screening Application

Congratulations! You've taken the first step in pursuing this life-changing surgery by either attending a seminar or by watching our educational video. If you feel you are eligible for bariatric surgery, the next step is to complete and submit the bariatric pre-screening application. Your health and well-being are our primary concerns, so you also will receive an extensive medical evaluation to determine if bariatric surgery is safe and appropriate for you.

If you would prefer a non-surgical approach to weight management, Mount Carmel Health System and Central Ohio Surgical Associates (COSA) have partnered to create a new program designed to combine weight loss medications alongside diet and lifestyle modification to promote improved health outcomes. If interested, simply fill out our online pre-screening application form and, if eligible, COSA will contact you to schedule your initial consultation.
