Diabetes Self-Management Education (DSME)
Managing diabetes can be complex. Many people find it a lot to handle without support. That’s why Mount Carmel offers the Diabetes Self-Management Program. It’s a chance to help you understand the demands of your disease by learning from the people trained to help you manage it.
Our program consists of individual appointments, group classes and support groups with a diabetes educator. We can give you the tools to control your diabetes — so it doesn’t control you.
The Diabetes Self-Management Program at Mount Carmel is certified by the American Association of Diabetes Educators and meets the National Standards for Diabetes Self-Management Education.
Eligibility
A referral from the provider managing the patient's diabetes is required to enroll in Mount Carmel’s Diabetes Self-Management Program.
Mount Carmel Diabetes Self-Management Education is generally covered by Medicare, Medicaid and many private insurance companies. We will submit the charges to the Mount Carmel billing office on your behalf. Your bill will be processed by the billing office based on the insurance information that you provide. Please bring insurance/Medicare/Medicaid cards to your first visit. We can submit to both a primary and a secondary payer when indicated.
You may need to pay a co-payment at your the first visit. You will receive a statement in the mail within a few weeks after each visit. If you have questions about your bill, you can call Customer Support at the billing office at 614-234-8888.
If you have insurance, we ask that you contact your insurance company directly prior to your first visit to verify coverage. When contacting them, the following information about our program may help them to determine coverage for you:
We are a hospital-based diabetes education program, recognized by the American Association of Diabetes Educators. Our staff consists of Registered Nurses, Registered Pharmacists and Registered Dietitians. Most are Certified Diabetes Educators.
Please have the following information available when you call your insurance company. Be sure to ask your customer service representative what your co-pay and deductible will be.
- The name of the service that you will receive is Diabetes Self-Management Education (DSME) or Diabetes Self-Management Training (DSMT)
- The billing form that we use is called a UB92 form
- The revenue code number that we use is 942
- The CPT codes may vary depending on your insurance carrier
- The CPT code for Private appointments will be either GO108, S9140 or 98960
- The CPT code for Group appointments will be either GO109, S9455, 98961 or 98962

If you have financial concerns and will have difficulty paying the total fee or the co-payment, please request an application for financial assistance. You can complete the form prior to your visit and bring it along with proof of income. We will submit the application to the Mount Carmel business office on your behalf. The billing office will decide if you are eligible for assistance. Once this decision has been made, you will be notified by mail in about 10 days.
Financial assistance can help you in a variety of ways. It can cover all or a part of your total charges or co-pays and deductibles if you’re eligible. Our comprehensive self-management program will provide you with the information needed to help you manage your disease with confidence. Classes are taught by registered nurses, dietitians and pharmacists. Class size is kept small enough so that you'll get plenty of individual attention.
We offer both daytime and evening classes. A support person is welcome to attend with you at no additional charge. The cost is usually covered by insurance.
Prior to attending classes, you will meet with an educator for a one hour session. The educator will schedule you for the classes that fit your needs. Classes consist of four, two hour sessions, held weekly. Topics covered in the individual session and each class session are listed below.

Individual session:
In this session you will meet with a diabetes educator individually. You will receive information about your own health risks. You’ll also learn about diabetes basics including your medications and checking your blood sugar.
Session I:
In the first group class you'll find out what diabetes is, why it happens, be introduced to carbohydrate sources, and what lifestyle changes you should consider making.
Session II:
In the second class, you’ll have a chance to try carbohydrate counting and read food labels. You’ll get in-depth information on how to make better food choices, how to take care of yourself when you are ill, and how to start an exercise plan. You’ll have help setting up a Success Plan for making important behavior changes.
Session III:
Session three continues the discussion on healthy eating by learning about protein, fats, salt and fiber. Diabetes medications are explained and you are introduced to how to prevent diabetes complications.
Session IV:
You will learn about healthy coping, preventing other diabetes complications, keeping up on changes in diabetes and setting goals.
Not everyone with diabetes uses injectable medication. For those who do, we’ve created a training program that covers everything you need to know about this important, effective therapy.
If you’re new to insulin, this training program is an ideal way to learn about its benefits and effects. This individual session will cover a host of topics, including:
- Why injectable medication therapy is necessary
- How to give an injection
- The importance of site rotation
- Type of insulin prescribed and how it works (onset, peak, and duration)
- The importance of timing injections with meals
- The importance of keeping blood sugar records to monitor progress
- When to report problems to your physician
Private consultations also are available for diabetes patients with immediate or special needs. You can schedule an appointment if you or someone you’re caring for:
- Has Type 1 diabetes
- Is unable to attend the Type 2 Basics class
- Has been diagnosed with gestational diabetes
- Has a vision or hearing impairment
- Needs the help of a translator
- Suffers from an intellectual disability or a developmental disability
- Private consultations cover many of the same topics as our comprehensive program, but the sessions are designed specifically to meet your individual needs.
Private consultations are available during normal business hours and are usually covered by insurance.
